But it is important that everyone understand the scope of the task awaiting RFKJr.” So, grab a cup of coffee, follow links, take notes if you need to and think carefully about your short and long term expectations of what are asking tasking Trump and Kennedy and Musk and Ramaswamy to do.

And this “*” (yep, that tiny asterisk at the very bottom) is the secret decoder ring to unraveling the nightmare of public health in the US. But first a snippet of this organization chart.
Which leads us to the Office of the Surgeon General:
“The U.S. Surgeon General is the Nation’s Doctor, providing Americans with the best scientific information available on how to improve their health and reduce the risk of illness and injury. The Surgeon General oversees the U.S. Public Health Service (USPHS) Commissioned Corps, an elite group of over 6,000 uniformed officers who are public health professionals. The USPHS mission is to protect, promote, and advance the health of our nation.”
And that, my dear readers, is where I lived for 30 years. But when I wore a USPHS uniform the Assistant Secretary(ASH) for Health unlike the current ASH, knew what a woman is.

This is a big part of the disfunction at DHHS: an agency that allowed and encouraged every wrong, every evil that emanated from the NIH/Tony Fauci manufactured SARS-CoV-2 virus and the COVID-19 that it caused was managed by RICHARD Levine who was at the helm of the Nation’s public health ship. The line authority of the ASH includes:

US Public Health Service
Office of the Assistant Secretary for Preparedness and Response
Office of Global Affairs
The Agency for Healthcare Research and Quality (AHRQ)
The Agency for Toxic Substances Disease Registry (ATSDR)
Centers for Disease Control and Prevention (CDC)
Food and Drug Administration (FDA)
Health Resources Services Administration (HRSA)
Indian Health Service (IHS)
National Institutes of Health (NIH)
Substance Abuse & Mental Health Services Administration (SAMHSA)
There are a couple of things that you need to understand while you dream about life without the FDA or CDC.
This reorganization will not happen in a matter of days or even weeks. But most certainly YUGE strides can be made in a year.
There are certain programs and agencies within those listed above that were created by Congress and can only be removed by Congress (see ATSDR).
As I describe below, Officers of the US Public Health Service can be moved around at the discretion of the Secretary of DHHS; Civil Servants cannot.
Substantial reorganization can be accomplished by the Secretary without the need for Congressional approval. Congress will have oversight and the democrats will pitch a fit but at the end of the rodeo RFKJr. will still be roping calves.
What is important about these agencies is that they are peppered with the 6,500 uniformed Officers of the USPHS who are subject to the same administrative rules as any other member of the uniformed services. That means that the Secretary (RFKJr.) acting through the ASH or the Surgeon General can move them anywhere he sees fit simply by cutting orders to reassign them. For example, IF he chooses to do so, Mr. Kennedy could reassign every officer to the Indian Health service or to the Public Health Emergency Response Strike Team (PHERST). Doing that would wreck havoc on the ability of the FDA to collude with he pharmaceutical industry, the CDC to issue “guidelines” forcing undeclared quarantines or “mask” wearing or the NIH to conduct gain of function research. That authority gives Mr. Kennedy a powerful tool to get the reorganization of the Nation’s public system rolling down hill. No federal employee unions need inquire.
A brief note: some USPHS officers serve in agencies outside of DHHS such as the EPA. However, those Officers are still subject to the authority of the Secretary, DHHS.
At the moment a series of executive orders signed by Joe Biden have installed DEI deep into the swamp. President Trump can negate those as soon as he takes the Oath of Office. HOWEVER, unless there is Congressional action to ban DEI from the government the next democrat administration will simply issue more executive orders. A global executive order can set the surgical removal of the cancer of DEI into motion but it must be followed by Congressional action to ensure that the DEI cancer never returns.
Removing the NIH, CDC, and FDA foundations ( see the Righting the Ship series here, here, and here) will require Congressional action as will extending the so-called cooling off period to 10 years for federal employees leaving government service and seeking employment in industries that they regulated , and the right of government employees to hold and profit from patents acquired by work funded by taxpayers.
The Secretary has broad authority to reorganize his operating Divisions and Offices; projects can be cancelled, money can be re-allocated or (hearsay of all hearsays) can be returned to the US Treasury.
As a very first step, Mr. Kennedy can order a review of all of the agencies and programs under his authority to find out which are there by Congressional order and which are there because someone in an agency convinced someone else that it was important. Once he has finished that he can identify duplicate functions within the various operating divisions. For example, the National Institute for Occupational Safety and Health (NIOSH) resides within CDC but it was established by Congress in the Occupational Safety and Health Act. One would think that NIOSH would be the sole agency for the Nation’s occupational safety and health research but that’s not case both the USPHS and CDC itself have occupational safety and health components separate and distinct from the Congressionally mandated NIOSH. This is known as jealousy.
A final word about the Commissioned Corps of the USPHS. As I wrote here, the Affordable Care Act (ObamaCare) in conjunction with Certificate of need laws in 35 states is forcing the closure of rural hospitals leaving rural Americans separated from nearby healthcare access. Within the USPHS there exists every category of Officer needed to fulfill the basic healthcare needs of millions of rural Americans. If Mr. Kennedy needs a place to park the 6,500 officers of the USPHS while he sets about the task of stopping our public health agencies from being public disease agencies rural America is waiting with open arms.
Then there is the issue of reorganization. Elon Musk and Vivek Ramaswamy have been tasked by President Trump to head DOGE, the “Department of Government Efficiency”. Though it is being called a “Department” it is not a real department of the federal government but rather a Presidential Advisory Commission. Perhaps, while RFKJr. is focusing on Making America Healthy Again he can coordinate the reorganization aspects of the task with Musk and Ramaswamy.
